Math Summer Camp Application Instructions
Printout and complete the application form. Make sure you clearly print all information. The camp(s) you select depend upon the placement test(s) you wish to retake.
Send completed application and check to:

Cost of the camp will be $110 per camp and covers all instructional materials. Upon receiving your application and check, you will be informed by your official UT email address if you have been accepted and you will receive additional information by email. If you are not accepted for the camp due to space limitations or not meeting pre-requisite requirements, your check will be returned. NO OTHER REFUNDS!

Notice
Space is limited in the camps, so apply early if you would like to attend! Application AND Full Payment must be received by July 23, 2008. Late Application Fee $25 per camp!
